1a.) I agree to pay $45 per week in return for a MOVEMENT. Group Training Membership that will give me access to unlimited group classes every week.

OR 1b.) I understand that my purchase of a 10 Class Concession Pass entitles me to attend 10 classes only.

2.) I understand that I can end my Group Training Membership at any time, however I am required to give 2 weeks’ written notice before cancelling. Payments can only be stopped 2 weeks from the date of notification.

3.) I understand that due to MOVEMENT. Functional Fitness’ ‘no lock-in contract policy’ if I cancel my Membership and then wish to return within the next 8 weeks I will be required to pay a $70 administration fee.

Waiver of Liability for Gym Use *

1.) I hereby understand and acknowledge that the training, programs and events held by MOVEMENT Functional Fitness may expose me to many inherent risks, including accidents and injury. 2.) I acknowledge the risk of injuries associated with participation in MOVEMENT Functional Fitness training sessions, including, but not limited to, falls, contact with other participants, and all other such risks present in a gym environment. 3.) I acknowledge that I am physically fit and capable of performing the physical activity I choose to participate in. 4.) I acknowledge my responsibility in communicating any physical and concerns that might conflict with participation in activity. 5.) In the event of an emergency, as deemed by the Gym in its sole discretion, I authorise MOVEMENT Functional Fitness to secure from any licensed hospital, physician or medical personnel any treatment deemed necessary for my immediate care. I agree to be responsible for payment of any and all medical services rendered and its associated costs. 6.) I agree MOVEMENT Functional Fitness are not responsible for any loss or theft of personal property brought to or left in the Gym. 7.) In consideration of all of the above, I agree, for myself and anyone entitled to act on my behalf, to hold harmless, waive and release MOVEMENT Functional Fitness, its employees and representatives from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in MOVEMENT Functional Fitness programs, classes and/or events. 8.) I have read and understand this Waiver of Liability. I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms.
